I understand by submitting this form and uploading my music video, images, biographical information, or any other form of media from me, I hereby grant permission to you to utilize my appearance in connection with the public broadcasting on Urban Sound Station TV. I hereby grant permission to Urban Sound Station to photograph me and to record my voice, performances, poses, acts, plays and appearances, and use my picture, photograph, silhouette and other reproductions of my physical likeness and voice, as described below. I have or will participate as indicated in the film/ television/radio project listed below (the “Program). I understand that Program may be produced and recorded for duplication and possible distribution throughout the United States & abroad. I acknowledge that the Program may be edited at producer’s discretion and used in whole or in part for broadcasting purposes in any type of media, in perpetuity throughout the world, including, but not limited to, non-profit cable television channels and/or digital platforms and/or websites. I consent to the broadcast of the Program and the publication of the Program in whole or in part after broadcast. I further consent to the use of my picture, photograph, silhouette and other reproductions of my physical likeness and voice, as well as my name, quotes and biographical information in connection with the Program, Program publicity and for Producer’s archival purposes. I further acknowledge producer’s right to crop or treat any such reproductions or recordings at its discretion. I hereby waive the right to any claim against producer, including any claim for fees, control of the aforementioned items or any right to inspect or approve such items or the use of such items. I expressly release producer, its licensees and assignees, from any privacy, defamation or other claims I may have arising out of broadcast, exhibition, publication or promotion of the Program. I have read the foregoing and fully understand the meaning and effect thereof and, intending to be legally bound.
